Auxiliary Pharmacy Mapping

Outcomes® uses Auxiliary Pharmacy Mapping (APM) to better serve patients in need of a Medication Therapy Management Service who have not received their services at their primary pharmacy. Patients who currently fill prescriptions at a lower-performing pharmacy are routed to an alternate pharmacy in their area that is interested in taking on additional MTM work. 

APM participants within the Outcomes Network have the option to complete Medication Therapy Management Services for additional area patients and to be paid, according to the plan's reimbursement rate, for completing these additional MTM services.

 

How APM Works

Outcomes looks for eligible patients to be mapped to the pharmacy and adds these additional patients to the pharmacy's Outcomes workflow. APM Pharmacies have up to 60 days to complete MTM services for APM patients to be paid for completing the additional MTM. Patients may refuse the pharmacy's offer to have their MTM services completed at the APM Pharmacy. Patients who accept an offer for MTM services from the APM Pharmacy will not be required to transfer their prescriptions to the APM Pharmacy. Participating as an APM pharmacy does not guarantee that your pharmacy will receive additional patients for MTM services. Patient availability is based on patient populations and pharmacy performance in your area. 

 

APM Pharmacy Requirements

• Complete and submit the form on this page to sign up to participate in APM.
• APM Pharmacies must continue to meet performance standards to continue to receive APM referrals.
• To participate as an APM Pharmacy, pharmacies must comply with the requirements outlined in the Outcomes Policy and Procedure Guide to ensure patients receive quality MTM services.
• Any actions found to violate these policies or procedures may result in the rejection of MTM claims and the removal of mapped patients from a pharmacy's Outcomes Patient List.
