To take advantage of the available MTM opportunities through Outcomes®, pharmacies, and clinics seeking to provide MTM Services from Outcomes must first complete a Network Participation Agreement by filling out the form and accepting the Terms & Conditions.

Retail Pharmacies

Most retail pharmacies are contracted via their chain organization or their PSAO. To get started, fill out the form below.

Chain Pharmacies

Chain, PSAO, or multi-location administrators can contract for their entire chain. To start the process of contacting your entire chain, request a multi-location agreement by email.

Pharmacists embedded within a physician clinic or health system can provide MTM services through Outcomes in an outpatient setting. To get started, fill out the form.
